Sizes, orientations and branding are all specified per order, and a custom insert size is available on request. A non-standard requirement is not an obstacle here — it is most of the work.
If your artwork or specification is still unresolved, say so when you enquire. It gets worked through before anything goes into production, and if something cannot be produced you will be told directly rather than quoted around.

Insert size and external size are not the same thing.
Most quoting mistakes start here. The insert is the card that goes in; the pouch itself is always slightly larger. Suppliers quote sometimes one and sometimes the other, rarely saying which.
Orientation matters more than people expect too — it decides which edge the lanyard slot sits on, so rotating your artwork is not the same as ordering the other orientation.
